PAYMENT: Dr. Ott accepts checks, cash, money orders and credit cards for payment. Please make checks payable to Derek A. Ott, M.D. and indicate in the memo section the date(s) of service and patient’s name. Please be aware that there is a $35 fee for any returned checks and denied credit card charges.
HEALTH INSURANCE: If you carry health insurance covering any service, it is your responsibility to carefully review your insurance coverage prior to your visit. Insurance benefits are a matter between you and your insurance company. Please be aware that Dr. Ott is NOT a participating member of ANY insurance panels for services provided in his private practice location. Therefore, he is not responsible for billing your insurance. Payment is required in full at the time of service. A billing sheet (i.e. super bill) appropriate for submission to your insurance carrier will be provided at the time of your visit. With this form, most insurance carriers generally provide some level of reimbursement.
